City of Birmingham Occupational Tax Refund Procedure

The City of Birmingham requires prior year occupational tax refund requests be submitted using the Affidavit Regarding Petition for Refund of Birmingham Occupational Tax form. The employee's supervisor must certify the completed document. The supervisor certification is a certification of the employee's work schedule only; it is not a certification of employment or salary-related information.
