As you might be aware, the IRB has begun to accept electronic submissions via the My Human Subjects module in IRAP as of Monday, June 26, 2017.

Because of this, the Department of Surgery has revised our submission process for Departmental Review and signature on the PORF form. You can now email your submission package to our contacts, Debbie Lowman and/or Lisa Clemons, instead of dropping off a hard copy of the submission package.

You can find the instructions and the revised PORF form here. The form can be completed electronically. The PI's signature is still required on this form, and it must be included in your submission package as a scanned PDF.

The IRB will continue to accept hard copy paper submissions until August 1; after that, all submissions must be made through the My Human Subjects module in IRAP.
