What are we asking of you?

We will ask your permission to allow your institution to release basic information to UAB research staff so that they can contact you. This can be done by signing a form.

Fill out a health questionnaire regarding your health and medical history. This questionnaire can be completed by mail (paper and pencil), phone, or online. If you wish to complete your health questionnaire by phone or online please call (855-903-2136) or email us (BMTStudy@peds.uab.edu).

If you had an autologous transplant (you received your own cells during the BMT) or if you did not have a BMT: Provide a saliva sample
For supporting this study, we would like to send you a $25 gift card and a treatment care plan.
