Requesting an Exemption from Immunization Reqiurements

Students requesting a medical or religious exemption from UAB immunization requirements must provide UAB Student Health Services (SHS) with a written request via the UAB Student Health Services Patient Portal. For medical exemptions, the student and/or parent/guardian (for minors) must sign and complete the downloadable “Medical Exemption Form” documentation supporting the medical necessity for exemption This form must be uploaded via the Patient Portal.

For religious exemption requests, the student and/or parent/guardian (for minors) must sign and complete the downloadable “Religious Exemption Form” outlining and affirming the student’s sincerely held religious tenants, beliefs, or practices.  This form must be uploaded via the Patient Portal.

Approved exemptions expire in 12 months and a request for both medical and religious exemptions must be renewed annually. SHS is responsible for reviewing each request and determining whether or not an exemption is warranted. The SHS Medical Director, or Associate Medical Director, is the final authority for UAB on exemption decisions.  All communication regarding exemption requests, including whether or not the exemption has been granted, will be via the SHS online patient portal.
