If you lose your passcode prior to taking the survey, immediately inform your manager so you can receive a replacement.

It is important for each of us to use this opportunity to provide honest feedback and hold leadership accountable to make improvements on your behalf. We listened to what you said in the 2015 surveys – and we acted. In response to concerns over pay, we were pleased to provide recent appreciation payments and merit increases. Efforts have also been made to address staffing level concerns, improve parking, and continue our focus on communication and opportunities for employee input. This year, we are once again asking for your feedback so we can continue to improve.
Remember, the survey is secure and completely confidential. Only our survey vendor has access to read responses. Passcodes are used in the Employee Engagement Survey to add another layer of protection and prevent employees from taking the survey multiple times.
