Important information for patients:

• Please include a legible copy of the front and back of the insurance card
• If you have any questions associated with a received billing statement, please contact the phone number listed on the mailed statement.  
• RUSH fees are not billable to any insurance company and must be paid up front by the patient.
• If your insurance company is not included in our list of contracted insurance companies, the following form can be used to determine if our testing services will be covered by your insurance provider:

• Please include a legible copy of the front and back of the patient’s insurance card
• The MGL will contact the insurance provider to inquire regarding the CPT code coverage for all samples submitted for insurance payment. The provider  will be contacted if: a) the insurance provider denies coverage of the requested codes b) supporting documents are required from the provider to confirm coverage c) a copay/deductible is expected to exceed $500. Please note: An insurance verification is not a guarantee of payment.
• RUSH fees are not billable to any insurance company and must be paid up front by the patient.
• If you would like the MGL to complete an insurance verification in advance of submitting a sample for testing, please complete the following form and submit it to our billing services coordinator.
