Applying to the School of Public Health
Students enrolled in the MPH, MSPH (ENH IHY concentration only), and DrPH programs must consent to and complete a criminal background check prior to starting their internship or practicum. Students will be restricted from enrolling in the courses associated with the internship/practicum until the background check is completed; failure to complete the internship/practicum in a timely manner will be considered “failure to progress” and may result in dismissal from the program.
The UAB School of Public Health is working with the third-party vendor, CastleBranch.com, to complete criminal background checks for students in the programs listed above. The cost of the background check is $45.75, which is payable by the student to CastleBranch directly.
Completing the Order for Your Background Check
- Download the student’s background check consent form.
- Students sign and return the consent form to Amanda Bosque in the SOPH Office of Student and Academic Services at abosque@uab.edu.
- Download the CastleBranch.com instruction sheet.
- Go to the CastleBranch website.
- In the Place Order box at the top right corner of the page, enter the package code IQ40oig.
- Please review the instructions and then click in the checkbox to continue.
- Follow the online instructions to enter all requested information to complete your order.
- If you are not a US citizen and therefore do not have a Social Security Number, please enter 111-11-1111 to proceed with your order.
- Enter your payment information. The cost of the background check is $45.75.
- Students will receive an order confirmation email with directions on how to log in to the MyCB CastleBranch account to check the status of their order.
- Students will receive alerts in the CastleBranch dashboard if additional information is needed.

Are Medical Students exempt from the background check?
No, MPH students are required to complete an internship.

If an international student completes his/her degree outside the US (including the completion of the internship) are they exempt from the background check?
Yes, international students completing their program outside the US are exempt from the background check requirement.

Are UAB employees (part-time and full-time) exempt from the background check?
Yes, UAB employees are exempt from the background check. Student employees are not exempt.

If a student completes their internship at their place of employment, can they be waived from completing the SOPH background check?
No, unless you are a UAB employee, you are not exempt from the background check.

Are students who complete their internship remotely exempt from the background check?
No, students who complete their internship remotely are not exempt from the background check.

Are students in the coordinated programs required to complete a background check If they have already completed one for another program (i.e., Social Work, or Physical Therapy)?
No. If a student in a coordinated degree program has completed a background check for their practicum or internship for that program, they are not required to complete the SOPH background check.
They will need to provide proof of completion of the background check.

If the student does not pass the background check, what happens?
It was decided that failure to pass the background check will not keep a student from receiving admissions or completing an internship and/or degree.
- If asked by the organization (internship site), the student would need to make the background check report available. Only the student can disclose an offense listed on the report to the organization.
- If the student cannot clear up the offense, it will limit the number of internships available to the student. Nevertheless, it will be up to the organization to determine if they will select the student for that intern.

We also offer a variety of scholarships for talented students who demonstrate the potential to make an impact in the field of public health. Eligibility requirements for specific scholarships can be found below.
All applications for UAB and School of Public Health scholarships should be submitted through BSMART, UAB’s comprehensive scholarship management tool.

Eligibility
The following are general eligibility requirements for applicants pursuing Master’s degrees in the School of Public Health. Note: all doctoral applicants should contact their department directly to discuss funding opportunities.
- Domestic students (U.S. citizens and permanent residents) with greater than 3.0 GPA are eligible for scholarships as of the first Fall semester in which they are enrolled. Apply by February 1 through BSMART for full consideration.
- International students with greater than 3.0 GPA are eligible for scholarships during their second year of enrollment. Students should apply by February 1 (Spring semester of their first full year) through BSMART for full consideration.
- Students applying for scholarships prior to enrolling in the UAB School of Public Health do not have to submit a letter of recommendation.
- All current students should submit one letter of recommendation from a UAB School of Public Health faculty member, unless this is your first semester of enrollment. In that case, you will be allowed to submit a letter of recommendation from a former college instructor, advisor, or an employer.
Some scholarships have additional eligibility requirements. They are noted in the descriptions below.

Dean's Excellence in Research Doctoral Scholarship
Eligibility criteria include:
- New doctoral applicants for the upcoming academic year
- All PhD and DrPH concentrations
- Doctoral student must be a full-time student; stipend will not allow the student to work outside of the SOPH
- Primary consideration is Merit and Potential to excel. This scholarship is intended to attract the highest quality applicants.
About
Beginning in 2023, each year, the Dean will award one fully funded doctoral scholarship in each department with a goal of recruiting, retaining, and graduating public health doctoral students of high distinction from the UAB School of Public Health. The scholarship includes up to four years as the period of coverage; exceptions may be made for extending beyond four years on a case-by-case basis. The scholarship includes full tuition, insurance, plus a stipend not lower than the current 12-month NIH NRSA level.
Each awardee will have research and teaching responsibilities. Each recipient will work with a faculty mentor from the UAB School of Public Health with expertise aligned with the student’s interests, who will in turn be the student’s primary advisor and mentor. Students will be expected to participate directly in research activities beginning in year one. Over a period of three successive course offerings (of the same course), the student will progress from Teaching Assistant, to Co-Instructor, to teaching the course.
Renewal
Each student will be required to form an Individual Development Plan (IDP) and will be reviewed for progress by the student’s Primary Research and Teaching Mentors, the Department Chair, and the Department’s Doctoral Program Director. Based on this review, the Department Chair will recommend to the Dean to continue or discontinue funding.
Notification of award
Recipients will be awarded in Spring/Summer 2023 for a Fall 2023 start date.

Association for Professional in Infection Control and Epidemiology (APIC) Scholarship
Eligibility criteria include 9 hours of enrollment per semester and:

- Epidemiology students with an interest in hospital epidemiology and/or infection prevention and plan to work in the State of Alabama following graduation.

Juanita Settine Memorial Scholarship
Juanita Settine was employed as a Research Associate in the School of Public Health Department of Environmental Health Sciences from 1986 until her death in 1999. She was the person faculty members and students would turn to when a problem or question arose regarding the laboratories. She was honored as Employee of the Month in 1991. This fund is used to honor Settine with plaques and scholarship awards.

Margaret J. Sullivan Scholarship
The award, which will go to a student with financial need, is named after Mary Sullivan’s mother, who died from lung cancer in 1985, two years before Mary earned her MPH at the UAB School of Public Health. A visit to UAB in 2016 inspired a renewed connection with UAB that ultimately led to Mary, who lives in Newtown, Connecticut, to establish a lasting legacy with the School of Public Health. Annual scholarships are awarded to deserving students in the School of Public Health.

Swatzell-Waterbor Health Equity and Diversity Endowed Scholarship
Kelley Swatzell, Dr.P.H., and John W. Waterbor, M.D., Dr.P.H. has made a generous gift to establish an endowed scholarship in the School of Public Health at the University of Alabama at Birmingham (UAB). The Fund shall provide support to students for whom affordability may be an obstacle and who might otherwise be unable to attend the University. Preference shall be given to students from underserved areas in Alabama or the Southeast; and students whose career intentions are to reduce health disparities in perinatal or cancer outcomes in these underserved areas. Secondary preference shall be given to students enrolled in dual degree programs at the University, to include the M.P.H. in combination with a graduate degree from the UAB Physician Assistant, Nursing, Social Work, Dentistry, Optometry, Medicine, or other health professional program.

The Sparkman Center for Global Health (Sparkman Center) and the Dean’s Office of the School of Public Health (SOPH) will provide travel funding for eligible SOPH students who are travelling internationally and attending an extracurricular academic/professional conference, training activity, Education Abroad program, or internship/practicum related to global health. These travel awards provide support for international educational experiences that enrich UAB SOPH students. The award may cover expenses related to transportation, lodging, poster printing, registration, and other international travel-related eligible expenses as specified by the travel regulations of the UAB Financial Affairs.

Katholi Endowed Scholarship in Biostatistics
This award was founded by Dr. Charles (1941 – 2024) and Ann Katholi in 2023. It was established to assist promising students to pursue graduate education in Biostatistics. This award acknowledges the legacy of Dr. Katholi who was an exceptional educator, researcher, and mentor throughout his career at UAB. Dr. Katholi began his career at UAB in 1970 as an Assistant Professor of Biomathematics. In 1982 he was named Associate Professor of Biostatistics and Biomathematics and in 1995 was appointed Professor of Biostatistics. He served as interim department chair for two years, 1997-1999 and was named Professor Emeritus in 2003.

How to Apply to Our Graduate Programs
STEP 1: Review Our Admission Requirements
Applying for a Master’s degree?
Please be prepared to submit the following:
- Online application: Complete and submit the online application form.
- Official transcripts: Provide official transcripts from every college-level institution attended. If you have completed coursework outside the United States, you must also provide a course-by-course foreign college transcript evaluation from any NACES accredited evaluation company. Transcripts should be translated to English, evaluated course-by-course, include the overall academic GPA, and be from an approved third-party company. A list of approved current members can be found on the NACES website.
- English proficiency test score: International applicants must submit an English proficiency test score, with a minimum score of 80 for TOEFL, 6.5 for IELTS, or 120 for Duolingo.
- Letters of recommendation: Submit at least two (2) letters of recommendation from academic or professional references. If your GPA is below a 3.0, please have at least one letter come from a reference who has taught you in a course.

- Statement of Purpose and Objectives
- Standardized test scores: The GRE is waived for all master’s degree programs. However, if an applicant has fewer than five years of full-time professional health care/public health practice experience AND a GPA below 3.0, we strongly encourage that applicant to submit a GRE score with their application.
- Minimum GPA: A minimum overall undergraduate GPA of at least a 3.0 average is preferred; however, applicants with positive GPA trend, public health experience, competitive GRE score, AND/OR an established graduate GPA (comprising of 12 or more graduate credit hours) are encouraged to apply.
Applying to a Doctoral degree?
Be prepared to submit all requirements listed above, as well as:
- The GRE: The GRE is required for all doctoral programs, EXCEPT for the following programs:
- PhD in Epidemiology
- PhD in Environmental Health Sciences
- PhD in Biostatistics
- DrPH in Biostatistics
- PhD in Health Behavior
For PhD in Epidemiology and Environmental Health Sciences applicants with less than five years of professional experience and GPA less than 3.0, including a GRE score is strongly encouraged to bolster the application.
NOTE: Application materials such as statement of purpose & letters of recommendation should be in the author’s own words, unenhanced by the use of generative AI, and citations used where necessary.

Background Check Requirement for MPH and DrPH Applicants
To ensure we are taking the necessary precautions to protect our community partners, the School of Public Health requires students enrolled in the MPH and DrPH programs that require an internship or practicum to consent to and complete a criminal background check. Students are responsible for completing the background check at least one term before registering for their internship or practicum. Failure to do so could prevent the completion of their internship or practicum degree requirement. Learn more about background checks.

School of Public Health policy requires submission of the completed online application, including ALL supporting materials, by the application deadline. Allow plenty of time for transcripts, letters of recommendation, and other documents to reach their destination.
STEP 3: Apply Online
If you’re applying to a stand-alone MPH, MSPH, or DrPH program, you will submit your primary application via sophas.org. If admitted, you will then complete a supplemental application to the UAB Graduate School. Instructions will be provided upon admission.
If you’re applying to any of our coordinated degree programs, Certificates, the MS in Biostatistics, an accelerated Master’s program (offered to current UAB undergraduate students only) or the PhD programs, submit your application via the UAB Graduate School.
