To ensure a safe and successful transition home, the UAB Department of Physical Medicine and Rehabilitation offers the PM&R Bridge Clinic, a follow-up program designed to maintain continuity of care after discharge.

Follow-up care helps prevent setbacks

The period following discharge can bring new challenges. Patients may be adjusting to medications, medical equipment, therapy schedules, or changes in daily routines. The PM&R Bridge Clinic helps address these concerns early, reducing the risk of complications and unnecessary hospital readmissions.

The clinic focuses on helping patients stay on track with their recovery while providing education, oversight, and coordination of care.

What patients can expect from the PM&R Bridge Clinic

Patients participate in a scheduled telehealth visit shortly after leaving the Rehabilitation Pavilion. Appointments are arranged before discharge to ensure timely follow-up.

During the visit, an Advanced Practice Provider (APP) specializing in rehabilitation medicine reviews the patient’s progress and recovery needs. The appointment may include:

  • Review of the discharge plan. Patients should have their discharge binder available.
  • Medication review and updates, with opioid medications continuing to be managed by the patient’s physician.
  • Evaluation of recommended medical equipment and assistance with access or availability if needed.
  • Education for patients and caregivers on care needs such as bowel or bladder management, wound care, or medications.
  • Coordination with home health services, outpatient therapy, and other medical appointments.

Caregivers play a key role in recovery

Recovery often involves caregivers, and the PM&R Bridge Clinic provides guidance to help them feel prepared and confident. Education and clarification during the visit can ease uncertainty and support a safer home environment.
