Sparks Clinic and Developmental-Behavioral Pediatrics

- OVERVIEW of SERVICES: Our clinic serves children for whom parents, health care providers, or other caregivers have concerns about development, behavior, social skills, and/or learning.
- For children 3 years old and under with developmental concerns: please make sure you have started a referral to Early Intervention by doing one of these things:
- Call Early Intervention Child Find at 1-800-543-3098 (or TTY 1-800-499-1816). Si habla español, puede llamar 1-866-450-2838.
- Fill out this Child Find Referral Form and fax it to 334-293-7393.
- Contact Help Me Grow Alabama at 833-939-0336; or fill out their intake form online at: https://helpmegrowalabama.org/ or email them at referrals@apcteam.org.
How to get an appointment:
Until further notice, we are only accepting referrals for children five years old and younger with a concern for Autism. All previously referred patients will remain on our wait list.
Providers:
- The Provider Referral Requirements contains the required information we request from the provider.
- All items from the Provider Referral Requirements MUST be included in your submission in order to have the referral reviewed for an appointment.
- "Patient Information Request Form" must be completed. "Auto submit version"
- "Patient Information Request Form" must be completed. "Printable version"
- Fax ALL required documents to 205-638-2526. (For Referrals and Patient Information)
- Fax: 205-975-2380 (For all other clinic information)
- If the required documents are not included, the referral will be returned.
- Once an appointment is scheduled, our scheduling staff will notify your office and the patient.
- All new patient appointments require a referral from a primary care physician.
- Complete the ‘Patient Information Request Form”.
- We currently have a scheduling waitlist of greater than one year. Our scheduling team will call you when an appointment is available.
- If you need help filling out the Patient Information Request Form packet, please call us at 205-996-4175.
- Proof of legal guardianship is required. Please email supporting documents to tiffanymarshall@uabmc.edu. If legal guardianship cannot be verified before the visit, the appointment will be cancelled and re-scheduled.
If you or your physician require any medical records to be released, please complete the form below and submit to tiffanymarshall@uabmc.edu.

This information is used to schedule customized appointments for the appropriate clinics and specialists.
Individual and/or group health insurance and Medicaid are accepted.
IMPORTANT INFORMATION FOR FAMILIES:
If your child does not currently receive services, i.e., speech/language, occupational therapy, physical therapy and is suspected of having a developmental delay, please contact Child Find at 1-800-543-3098 for Alabama's Early Intervention Services (for children 0-3 years old), or the pre-school special education coordinator at your local school system for those children who are ages 3 and over but not yet in kindergarten. If your child is in school but not receiving services, and you believe they should, please request your school system evaluate them. Do not wait on your appointment with us.
